Augments are permanent passive upgrades. As you level and prestige, the game periodically offers you a hand of augments to choose from — pick one, and it stays with your character for good. Over a long career you end up with a stacked collection of Common and Utility augments plus one each of the rarer, unique tiers, and together they're a big part of what makes one character actually feel different from another.
You never grind for an augment offer directly. Every offer is unlocked by hitting a level or prestige milestone; the only randomness is which augments show up inside the offer you're handed.
The five tiers
Augments come in five tiers, and the tiers split cleanly into two behaviors:

Common and Utility are the stackable tiers — every copy you pick adds on top of the ones you already have. Utility in particular is the dedicated resource-regen track: its augments are built around mana, stamina and other regenerating resources, and stacking more of them compounds that regen.
Elite, Legendary and Mythic are unique. You can hold exactly one of each augment in these tiers, so picking one from an offer is a real choice — and if you want to change your mind later, that's what rerolls and blacklists are for.
